Job Description
Essential skill and experience:
- Graduate of an approved school of nursing; current Tennessee license to practice as a registered nurse or licensed practical nurse.
- Proficient reading, writing, grammar, and mathematics skills; proficient interpersonal relations, and communicative skills; auditory and visual skills; satisfactory completion of post-offer, pre-employment physical as required.
- Time management: the ability to organize and manage multiple priorities
- Proficient computer skills including word, excel and MDS software
Preferred skill and experience:
- A minimum of two year’s prior experience in long-term care preferred.

Essential Duties:
- Coordinates with DON/ADON to monitor and minimize the use of resident restraints.
- Determine the kind and amount of nursing care needed to meet the needs of the resident and to achieve the objectives of the nursing home for the resident care plan and MDS.
- Tracks falls and report findings in Quality Assurance meeting.
- Coordinates with Social Services and Nursing Department to complete PAE/PASRR if needed.
- Coordinates with Admissions to create financial reports for potential new admissions into Skilled rehab.
- Coordinates with the Interdepartmental team to monitor resident medication and reduction of psychotropic medications.
- Works with financial department and MCO ( Managed care organizations) for payment approval and appeals
- Verify resident histories and tracking for discharge and re-entry for MDS.
- Input admission and discharge data into MDS.
- Facilitates patient care plan meetings providing resident issues and goals.
- Schedules Patient Care Plan and MDS meetings and sends appropriate notice to families.
- Ensure accuracy of the input data for electronic submission.
- Active member of organizations committees and Eden Employee Training.
- Monitors and tracks falls/incidents and reports in QA
- Provides documentation and input on quarterly QA/QI.
- Coordinates processes with MCO.
- Monitors falls and incident/accidents and coordinates for proper interventions and reports to QA monthly.
- Works as a member of the IDT to coordinate and adhere to deadline submissions of MDS data.
- On call for Admissions/referrals.

- Fully understand all aspects of residents’ rights, including the right to be free of restraints and free of abuse. Responsible for promptly reporting to the director of nursing, or administrator incidents or evidence of resident abuse or violation of residents’ rights, following abuse procedure.

- Work environment: While performing the duties of this job, the employee is exposed to odors, body excretions, and infections. Subject to bruises, scratches etc. from patients. The noise level is usually moderate. Significant amount of time at computer inputting data.
